What Your Annual Physical Doesn't Test (But Probably Should)
You go to your annual physical. The doctor runs your bloodwork, glances at the results, and tells you everything looks "normal." You walk out relieved, and then walk back into a life where you're exhausted by 2 p.m., foggy at work, slow to recover from workouts, and not quite feeling like yourself. If "normal" labs and not-normal symptoms sound familiar, you're not imagining the disconnect. A standard physical is built to catch disease, not to find out why you feel off, and those are two very different jobs.
"Normal" Doesn't Mean "Optimal"
Lab reference ranges are designed to flag clear-cut medical conditions. A result can sit comfortably inside the "normal" range and still be far from where your body functions best, especially if you're over 40 and your numbers have been drifting for years. That's why people so often hear "everything looks fine" while dealing with fatigue, brain fog, stubborn weight gain, low motivation, or poor sleep.
The gap usually isn't in the tests that get run. It's in the tests that don't.
Tests That Often Get Skipped
A routine physical commonly covers cholesterol, basic blood sugar, a metabolic panel, and a TSH thyroid screen. These are useful, but they leave out several markers that tie directly into the symptoms people actually complain about:
Free and total testosterone. Total testosterone alone can hide the fact that very little of it is actually available for your body to use. A full thyroid panel. TSH by itself can miss early or subclinical thyroid problems. Free T3 and free T4 show how much active hormone your tissues are actually getting, which matters for energy, weight, and mental clarity.

Cortisol patterns. One morning cortisol number can miss a flattened or reversed daily rhythm, which is the pattern linked to poor sleep, stress-related inflammation, and slow healing.
Fasting insulin. Fasting glucose can stay normal for years while insulin quietly climbs. Elevated insulin shows up long before blood sugar does and affects weight, energy, and brain function.
Inflammatory markers like hs-CRP. Chronic low-grade inflammation doesn't hurt in an obvious way, but it undermines recovery, hormone signaling, and cognition. A high-sensitivity CRP test is a simple way to see it.
B12, iron/ferritin, and vitamin D. Deficiencies in any of these can mimic hormone problems, and they're easy to miss if they aren't specifically ordered.
Why These Markers Work Better Together
No single number tells the story, because these systems influence each other. Poor sleep raises cortisol, high cortisol suppresses testosterone, inflammation interferes with hormone signaling, and low oxygen delivery to tissue slows repair across the board. Where Hyperbaric Oxygen Therapy Fits In
Better testing is what makes a better plan possible, and for many people that plan includes hyperbaric oxygen therapy (HBOT). When your labs point to chronic inflammation, slow recovery, or a stress-driven cortisol pattern, HBOT addresses the environment those problems live in. By dissolving significantly more oxygen into the blood plasma, it improves oxygen delivery to tissues that are inflamed or under-supplied, and it has a documented anti-inflammatory effect.
That's also why HBOT pairs well with hormone optimization rather than competing with it. Hormone therapy restores the signals; HBOT improves the conditions those signals work in. Your lab results are what tell us which piece, or which combination, makes sense for you. HBOT isn't the right answer to every abnormal result, and part of a good evaluation is knowing when something else, like a nutrient deficiency, a thyroid issue, or sleep apnea, is the real driver.
How to Get the Testing You Actually Need
A few practical steps make the next appointment more productive:
Write down your symptoms and when they started. Patterns over weeks or months matter more than any one bad day.
Ask specifically for the markers above rather than hoping they're included. Many are simply not part of a default order.
Request your actual numbers, not just "within normal limits." Trends over time are often more telling than a single result.
Work with a physician who treats the whole picture, not just whichever value falls outside the printed range.

Feeling "off" when your labs say you're fine isn't a sign that you're overreacting. It's usually a sign that the right questions haven't been asked yet.
