Understanding Radiation Tissue Damage
Radiation therapy is one of the most effective tools in cancer treatment, used in roughly half of all cancer patients. But ionizing radiation doesn't distinguish precisely between tumor cells and surrounding healthy tissue. Over time, the vessels and connective tissue in the treatment field become progressively damaged, leading to a state of tissue hypoxia (oxygen deficiency) and fibrosis. This condition — known as radiation-induced tissue injury or osteoradionecrosis when bone is involved — can appear months or even years after treatment ends.
The mechanism is a self-perpetuating cycle: radiation damages blood vessels, reducing oxygen supply; depleted oxygen causes cells to die; dying tissue triggers more fibrosis, further blocking oxygen delivery. Standard treatments often offer only symptomatic relief without breaking this cycle.
Radiation injuries commonly seen at NorCal Hyperbarics include:
Radiation cystitis: Painful urination, urgency, and hematuria (blood in urine) following prostate, bladder, or gynecologic cancer treatment.
Radiation proctitis: Rectal bleeding, pain, and bowel dysfunction after colorectal or prostate treatment.
Osteoradionecrosis of the jaw: Jawbone death following head and neck or oral cancer radiation, often triggered by dental procedures.
Soft tissue radionecrosis: Non-healing wounds or tissue breakdown in the breast, chest wall, or pelvis following radiation to those areas.
Radiation-induced brachial plexopathy: Nerve damage in the shoulder and arm following breast or lung cancer treatment.
How HBOT Reverses Radiation Tissue Damage
HBOT directly attacks the underlying cause of radiation injury — tissue hypoxia — rather than simply managing symptoms. Breathing 100% oxygen under pressure of 2.0 to 2.5 atmospheres delivers plasma oxygen concentrations 10 to 15 times higher than normal, saturating even the most ischemic, fibrotic radiation-damaged tissue.
At these elevated oxygen levels, HBOT stimulates the sprouting of new blood vessels (angiogenesis) within the previously hypoxic radiation field. This neovascularization is the key mechanism: as new capillaries grow back into the damaged tissue, normal oxygen delivery is gradually restored, allowing cells to resume their repair functions, fibrosis to soften, and chronic wounds to close. HBOT also mobilizes stem cells from bone marrow, increases nitric oxide production (which promotes vascular relaxation), and reduces the chronic inflammatory signaling that sustains radiation injury.
For osteoradionecrosis, HBOT is used both before and after surgical intervention (the "Marx protocol") to optimize tissue vascularity prior to procedures and accelerate recovery afterward. Multiple studies and the Undersea and Hyperbaric Medical Society (UHMS) — which officially recognizes delayed radiation injury as an approved HBOT indication — support this combined approach. Typical protocols range from 20 sessions for soft tissue injuries to 40 sessions or more for osteoradionecrosis.
