Concussions are one of the most common — and most misunderstood — injuries in youth and adult sports. Most people recover within a few weeks, but a meaningful percentage go on to develop Post-Concussion Syndrome (PCS): lingering headaches, brain fog, fatigue, and mood changes that can drag on for months. For families and athletes stuck in that gray zone, hyperbaric oxygen therapy (HBOT) has become a frequent question. Here's what the research actually says.

What Post-Concussion Syndrome Looks Like

PCS affects roughly 15–30% of people after a mild traumatic brain injury, and it can be disruptive well beyond the original injury — affecting school performance, athletic return-to-play timelines, sleep, and mood. Standard care usually includes rest followed by graded return to activity, physical therapy, and cognitive rehab. But when symptoms persist for months, treatment options become limited, which is part of why HBOT has drawn growing research interest.

Why Researchers Are Studying HBOT for Concussion

HBOT works by significantly increasing the amount of oxygen delivered to tissue, including the brain. In the context of a concussion, the theory is that added oxygen availability can support the brain's own repair processes — reducing inflammation and helping the metabolic recovery of neurons that were disrupted, but not permanently damaged, by the injury.

What the Evidence Actually Shows

This is an area where the research is genuinely evolving, and the results have been mixed:

  • Pediatric studies are encouraging. A randomized, sham-controlled trial in children with persistent post-concussion syndrome found meaningful improvements in cognitive function, memory, executive function, and behavioral symptoms after 60 HBOT sessions compared to a sham treatment group.

  • Adult studies with childhood TBI history show promise. Research on adults who sustained a brain injury in childhood found HBOT was associated with improved cognitive outcomes years later, though this was an observational study rather than a randomized trial.

  • Military and veteran trials have been more inconsistent. Department of Defense-funded randomized trials in service members with PCS have produced mixed results, with some finding no significant benefit over sham treatment — a reminder that results can vary by population, injury type, and treatment protocol.

The takeaway: HBOT is not a guaranteed fix for PCS, and it isn't currently a standard first-line treatment. But for patients who haven't improved with conventional care, particularly in pediatric and civilian populations, it's a legitimate area of ongoing research worth discussing with a physician.

What This Means for Athletes and Families
