HBOT for Autoimmune Conditions: What Patients Are Asking About Right Now
If you live with an autoimmune condition, you know the routine: flares that arrive without warning, fatigue that sleep doesn't fix, joint pain or gut trouble that changes by the week, and a medication list that never seems to get shorter. It's no surprise that many people with lupus, rheumatoid arthritis, Hashimoto's, Crohn's, and similar conditions start looking for something that works alongside their current care. One question keeps coming up in our office and in online searches: can hyperbaric oxygen therapy (HBOT) help?
Here is an honest look at what we know, what we don't, and who it may make sense for.
Why People Connect HBOT With Autoimmunity
Autoimmune conditions share a common thread: an immune system that stays switched on when it shouldn't, driving chronic inflammation. That inflammation is why so many of these conditions come with overlapping symptoms like fatigue, brain fog, pain, and slow recovery.
HBOT enters the conversation because of what it does in the body. Inside a pressurized chamber, you breathe near-100% oxygen, which dissolves far more oxygen into your blood plasma than normal breathing can. Researchers are studying whether that extra oxygen can help calm inflammatory signaling, including markers like IL-6 and TNF-alpha, support mitochondrial function, and improve oxygen delivery to tissue that's been inflamed or under-supplied. These are the same pathways we've covered in our posts on chronic stress and healing and sleep, hormones, and oxygen delivery.
What the Research Actually Shows
This is where honesty matters most. Interest in HBOT for rheumatic and immune-mediated conditions is growing, and recent reviews have started to examine both the mechanisms and the clinical results. But the evidence is still early:
Most of the research is small. Much of what exists comes from small studies, case series, and mechanistic work rather than large randomized trials.
Results vary by condition. Some conditions have encouraging early signals, while others, such as multiple sclerosis, have not shown convincing benefit in published reviews.
HBOT is not an FDA-approved treatment for autoimmune disease. It is approved for a specific list of conditions, including non-healing diabetic wounds and radiation tissue injury. Use for autoimmune conditions is considered off-label and investigational.
In other words, anyone promising that HBOT will "reverse" or "cure" an autoimmune condition is overselling it. What it may offer is a way to support the body's inflammation and recovery burden as part of a broader plan.
Where It May Fit
For patients we see, the most realistic goals are supportive ones:
Lowering the day-to-day inflammatory load that contributes to fatigue and pain
Supporting tissue repair and recovery between flares
Easing brain fog and low energy, which are among the most frustrating symptoms of autoimmune disease
Complementing, not replacing, the care your rheumatologist, endocrinologist, or gastroenterologist already provides
It also helps to look at the full picture. Autoimmune symptoms often overlap with hormone imbalance, thyroid dysfunction, nutrient deficiencies, and chronic stress, which is why a comprehensive lab evaluation is a better starting point than any single therapy. Sometimes what looks like a flare has another driver we can address directly.
Safety Questions to Ask First
HBOT isn't right for everyone, and a screening visit matters, especially if you take immune-modifying medications. Make sure the clinic reviews:
Every medication and supplement you take, since a small number of drugs interact with HBOT
Your lung and ear history, including any recent chest or ear surgery
How HBOT fits with your current treatment plan, ideally in coordination with your specialist
Common side effects are mild, such as ear pressure and temporary fatigue, but they're worth discussing upfront.
The Bottom Line
Autoimmune conditions are complex, and no single therapy is a cure-all. HBOT is not a replacement for your medical care, and the research is still catching up to patient interest. But for people dealing with chronic inflammation, persistent fatigue, and slow recovery, it can be a reasonable, physician-guided addition worth discussing, especially when it's paired with proper testing and a plan built around you.
