Urinary Symptoms

WHAT IS IT

Understanding Radiation Cystitis

Radiation cystitis is a delayed complication of radiation therapy to the pelvis, arising when the bladder receives significant radiation dose during treatment of prostate cancer, bladder cancer, cervical cancer, endometrial cancer, rectal cancer, or other pelvic malignancies. The bladder's proximity to the prostate and other pelvic structures makes it unavoidable collateral territory in many pelvic radiation fields, even with modern intensity-modulated radiotherapy (IMRT) techniques.

Radiation cystitis develops in a biphasic pattern. An acute radiation reaction occurs during or immediately after treatment, causing temporary urgency, frequency, and dysuria that typically resolves within weeks. A more significant and often permanent late radiation injury emerges months to years later — as the progressive radiation endarteritis (vascular damage) produces a chronically hypoxic, fibrotic bladder wall with fragile, easily-bleeding mucosal telangiectasias. This late radiation cystitis — particularly its hemorrhagic form — is a Medicare-approved HBOT indication and the primary focus of treatment at NorCal Hyperbarics.

How HBOT Heals Radiation-Damaged Bladder Tissue

Radiation cystitis is caused by the same fundamental mechanism that drives all late radiation tissue injury: progressive endarteritis and fibrosis that reduces blood flow, creating a chronically hypoxic environment in the bladder wall. Radiation-damaged endothelial cells in bladder vessels proliferate and occlude the vessel lumen over months to years, progressively ischemic the urothelium that lines the bladder. This ischemic mucosa becomes fragile, develops telangiectasias (dilated, thin-walled superficial vessels) that bleed easily, and loses the barrier function that protects underlying tissue from urine. The resulting cycle of ischemia, mucosal breakdown, and bleeding is self-perpetuating without intervention that addresses the vascular supply failure.

HBOT breaks this cycle directly. At 2.0 to 2.5 atmospheres of 100% oxygen, HBOT achieves tissue oxygen concentrations sufficient to trigger new blood vessel formation in the hypoxic bladder wall. This angiogenesis — stimulated through VEGF upregulation and endothelial progenitor cell mobilization — gradually replaces the obliterated radiation-damaged vasculature with a new capillary network, restoring mucosal oxygenation and allowing the urothelial barrier to regenerate. As the mucosal integrity is restored, the fragile telangiectatic vessels that were the source of bleeding undergo remodeling, hematuria resolves, and the urgency and pain symptoms driven by mucosal irritation diminish.

HBOT for radiation cystitis is supported by multiple clinical series and is covered by Medicare. The most commonly cited resolution rates are in the range of 76 to 90% for complete or partial resolution of hemorrhagic symptoms. Protocols typically involve 30 to 40 sessions of 90-minute treatments, with ongoing assessment of symptom response.
