Understanding Autism Spectrum Disorder (ASD)
Autism spectrum disorder (ASD) is a neurodevelopmental condition characterized by persistent differences in social communication and interaction, and by restricted, repetitive patterns of behavior, interests, and activities. It is diagnosed across a spectrum of severity and functional ability — from individuals who are minimally verbal with significant support needs to those with strong verbal skills who struggle primarily with social communication and sensory differences. ASD affects approximately 1 in 36 children in the United States as of the CDC's most recent estimates, and it is approximately four times more common in boys than girls.
The cause of ASD is multifactorial, involving complex interactions between genetic predisposition and environmental factors. Current research has identified neuroinflammation, oxidative stress, mitochondrial dysfunction, gut microbiome dysbiosis, and cerebral hypoperfusion as biological characteristics present in subsets of individuals with ASD — each of which may be responsive to targeted interventions including HBOT.

What the Evidence Says About HBOT and Autism
The biological rationale for HBOT in ASD is grounded in two findings that have been replicated across multiple research groups: first, SPECT cerebral blood flow imaging studies consistently document hypoperfusion in the temporal and frontal regions of the brain in ASD subjects; and second, postmortem brain studies and cerebrospinal fluid analyses document elevated neuroinflammatory markers — activated microglia, elevated cytokines, and oxidative stress biomarkers — in a significant proportion of ASD brains. These biological abnormalities, rather than the behavioral symptoms themselves, are the proposed targets of HBOT.
At low to moderate pressures (typically 1.3 to 1.5 atmospheres in the ASD literature — lower than the pressures used for wound care or TBI), HBOT increases cerebral blood flow to hypoperfused brain regions and delivers elevated oxygen concentrations to neuroinflammatory tissue. A double-blind randomized controlled trial published in BMC Pediatrics found that children with ASD who received HBOT showed significant improvements compared to controls in overall functioning, receptive language, social interaction, and eye contact as rated by blinded observers. Improvements in sensory processing and stereotypy were also noted.
We want to be direct with families: the evidence base for HBOT in ASD, while biologically plausible and encouraging, is not as established as it is for conditions like diabetic wounds or carbon monoxide poisoning. A Cochrane systematic review found insufficient evidence to draw definitive conclusions about HBOT for autism. The children who appear to respond best are those with documented cerebral hypoperfusion, significant GI dysfunction, or elevated inflammatory biomarkers. Dr. Toth provides a thorough initial evaluation and sets realistic expectations before recommending a treatment course. HBOT is always used as an adjunct to — never a replacement for — established behavioral, educational, and speech therapies.
